Position Summary
The Case Manager plays a critical role in supporting justice-impacted individuals as they navigate successful reentry into the community. Under the limited supervision of the Program Director, the Case Manager provides client-centered, trauma-informed case management services focused on job readiness, identification documentation, housing stability, mental and physical health access, and overall wellness.
This role involves assessing client needs, developing and implementing individualized service plans, coordinating wraparound services, and advocating for participants to ensure equitable, effective, and dignified care throughout the reentry process.
Primary Responsibilities
- Verify participant eligibility using the CDCR Inmate Locator tool
- Serve as the primary point of contact for clients’ basic needs and reentry support
- Conduct comprehensive client assessments and develop individualized service plans
- Provide and document 16 weeks of hour-long direct services per program participant
- Coordinate wraparound services, including employment support, housing resources, mental and physical health referrals, and identification assistance
- Transport clients to appointments as needed
- Monitor client progress, update case notes, and determine appropriate case closure
- Advocate for clients in navigating social services, health systems, and government agencies
- Support clients in achieving autonomy related to education, employment, and stable housing
- Facilitate care coordination and information sharing across internal teams and external partners
- Address motivational, psychosocial, and behavioral barriers to success
- Maintain accurate records, complete required documentation, and generate statistical and program reports
- Promote quality, cost-effective, and client-centered interventions
- Build strong working relationships with program staff, leadership, and community partners
- Adhere to all professional, ethical, and organizational standards, policies, and regulations
- Perform other related duties as assigned
Essential Job Requirements
- Associate degree or equivalent combination of education and relevant work experience
- Two (2) to five (5) years of experience working in a nonprofit environment (preferred)
- Experience providing or coordinating wraparound services
- Strong problem-solving skills and a strategic, solutions-oriented mindset
- Excellent verbal, written, and interpersonal communication skills
- Strong organizational and time-management abilities
- Proficiency in Microsoft Office and general computer/technology systems
- Ability to work independently while collaborating effectively with a team
Additional Requirements
- TB Test (required)
- CPR and First Aid Certification (provided by employer if needed)
- Valid Driver’s License and ability to transport clients
