Job Description: This individual facilitates a multi-disciplinary review of case files and ensures that the required number of clients managed is maintained.
They work closely with the team to integrate referrals(intake), reports, and expand the reach of the program through client service provision and assist with the supervision of team members.
They Assess client needs and develop and maintain corresponding Individual Service Plans (ISP). They administer assessments, facilitate developmental activities and workshops, assist in planning and implementing program and community events, and participate in community engagement outreach efforts.
They maintain case files, input all required information in the online database system, and attend all mandatory meetings and trainings.
They maintain a combined case load of at least forty (40) clients throughout the contract period.
