    Position Title: Social Care Network Coordinator

    Working Title: SCN Coordinator

    Classification: Full time, hourly, non-exempt, benefits eligible

    Schedule: 35 hours per week, Monday through Friday, 9am-5pm with required breaks

    Primary Role:

    The Social Care Network Coordinator (SCNC) is responsible for effectively supporting the organization’s engagement with Hudson Valley Care Coalition Social Care Network (HVSCN) within the assigned target areas and reach, and under the leadership of the Family Programs Director. Priority focus will be the screening and referral of clients, provision of direct services through communications with clients received through the HVSCN, and case support as needed to ensure clients processed through the HVSCN are appropriately served. This will require the SCNC work cooperatively with the Family Program Director to determine work priorities, evaluate client needs and deliver services, and provide reporting and other information internally and through the UniteUs database platform. This position will require significant daily interface with the UniteUs platform and telephone communication. This work will require working effectively with other staff and areas of NECC to accomplish outcomes.

    Leadership Responsibilities:

    The SCNC will:

    • Respond in real time to all contacts received by NECC requesting services related to the HVSCN network via telephone, email and/or the UniteUs platform
    • Serve as the SCN Screener role to screen and engage within the UniteUs platform, preparing contacts for the next stage of engagement, SCN Navigation
    • Assess client needs and strive to assist/empower the meeting of those needs within the guidelines of the program and usage of the appropriate consents provided within UniteUs screening
    • Serve as the SCN Navigator role for clients that are able to proceed to the next stage, to assess/ process for service provision; using the UniteUs platform, clients will receive referrals to external (not NECC) or internal (NECC) service providers as trained by the HVSCN and UniteUs administrators
    • Carry out internal UniteUS steps through which HVSCN service provision authorizations would be obtained and automated invoicing for Screening and Navigation services are completed
    • Conduct intakes, complete documentation, transmit paperwork to clients and upload any necessary documentation to the UniteUs platform
    • Serve as the NECC internal SCN Care Manager processing clients for Level 1 and Level 2 services
    • Provide accurate and complete data entry consistently via the UniteUs platform
    • Communicate regularly with the Family Program Director to discuss program progress and resolve problems
    • Complete all initial and ongoing trainings as required
    • Represent NECC at HVSCN meetings if appropriate for subject matter
    • Participate in professional development and training annually
    • Attend monthly NECC staff meetings
    • Follow all NECC Policies and Procedures
    • Maintain regular attendance
    • All other duties as assigned

    Program Delivery:

    The SCNC will:

    • Conduct SCN Care Management Level 1 and Level 2 service provision as needed
    • Engage Family Program Director, case management staff and food program staff as needed
    • Provide program delivery back up support to case management staff, food program staff and transportation staff as needed and time permitting

    Qualifications:

    • Bachelor’s degree in social work, human development, human services, or related area
    • AND a minimum of 2-years of experience in a position similar to the one described
    • Bilingual/Spanish speaking required (Note in cover letter level of fluency)
    • Knowledge of social programs and services
    • Ability to use technology, including Microsoft Word and online email and calendar programs
    • Satisfactory background check through central database and DMV Record check

    ADA Requirements:

    • Ability to lift a 50-pound box
    • Ability to walk up and down stairs
