    Descripción

    • Provide consistent supervision to case managers and navigators to support respective deliverables and performance related provision of services, outreach strategies, contract deliverables and training needs.
    • Collaborate with DCHH to coordinate onsite and field-based Case Management service provision to OnPoint NYC participants,
    • Work closely with Director of Clinical Services and Supportive Services, and Holistic Health to ensure OnPoint’s deliverables are met across supportive service contracts.
    • Conduct regular individual and group supervision of case managers and navigators to maintain quality service provision and ensure adherence to OnPoint NYC policy and procedure.
    • Coordinate outreach activities for Case Managers to increase engagement of potential participants, including waivered-sites, community events, and with the OPST.
    • Attend case conferencing and coordinate follow-up responsibilities for case managers department.
    • Ensures facilitation of timely access to services for participants; coordinates follow-up to promote successful completion of linkages and referrals.
    • Coordinates linkages and follow-up made for Buprenorphine Induction, Medication Assisted Treatment (MAT) programs, Addiction Treatment, HIV/HCV/STI treatment and care
    • Reviews documentation of services to ensure provision of care is high quality, audit worthy, and culturally appropriate
    • Coordinates facilitation of seamless transfer of care from lower threshold programs into onsite confidential services
    • Collaborate with Volunteer and Training Coordinator to ensure all training requirements are met across supportive services contracts.
    • Responsible for group programming schedule in East Harlem; schedules groups, coordinates coverage, reviews documentation, and ensures high-quality group facilitation
    • Support case management staff with completion and submission of documentation into appropriate reporting systems, ensure all staff are adequately trained for Welligent, SND app.
    • Coordinate with the Director of Clinical Services and Supportive Services and Holistic Health, Data and Evaluation team members to review trends, successes, and challenges related to service provision, patterns of engagement, and care coordination services.
    • Proactively troubleshoots service deficits and contract challenges as they arise.
    • Submit quality reports on time, on a monthly basis to director-level staff as needed for contract management, foundation reporting, and communications.
      • Ensure adequate staff coverage and proactively manages staff absences and/or position vacancies to minimize disruption to service provision.
      • Review timesheets for case management department
    • Perform administrative duties related to payment requests, supply inventory, and ordering
    • Monitor calendar for time and attendance notices added by the staff; ensure calendar protocol is correctly followed by staff.
    • Review and edit timecards regularly.
    • Ensure PTO is entered correctly by staff in ADP.
    • Inform staff about PTO/sick time balances as necessary.
    • Perform other related task and responsibilities as related

    Qualifications:

    • Minimum 5 years of experience working in a relevant field/case management experience in NYC required
    • Minimum of 2 years management experience; managing staff in a harm reduction setting a plus
    • Commitment and understanding of the harm reduction philosophy in both service provision and management; familiarity with HIV and HCV service provision, social service agencies and health care services required
    • Strong proven leadership skills a must. Ability to motivate staff, provide constructive and supportive feedback, and adapt approach to match various learning styles and abilities
    • Should be organized, self-motivated, creative, and committed to working in a diverse environment
    • Ability to coordinate multiple tasks simultaneously in a fast-paced environment
    • Pragmatic and competent problem solving and conflict resolution skills
    • Ability to work with diverse communities (gender, ethnic, age, language, political)
    • Ability to master a wide range of information and communicate accordingly
    • Excellent communication skills (written and oral); timely follow up on tasks for staff and supervisor
    • Great attitude and people skills are a must
    • Bilingual (English and Spanish) required
