    As a Contract Housing Facilitator, you will be the first point of contact for individuals seeking CHN housing. You’ll assess applicants, support them through the qualification process, and collaborate with community partners to connect people to appropriate housing—ensuring all requirements are met along the way.

    What you will be doing in the role:

    • Applicant Assessment & Screening- Conducting housing assessments, reviewing applicant files, and addressing barriers to placement to connect individuals with appropriate CHN housing. Coordinating with referral systems and partner agencies, responding promptly to inquiries, and performing timely assessments in the community to ensure effective housing access and support.
    • Administrative & Clerical Management- Maintaining accurate files and records to meet compliance requirements and respond to audits, ensuring all documentation supports subsidy and housing eligibility. Monitoring waitlists, preparing residents for move-in by verifying documentation, and collecting program data, generating service utilization reports, and submitting grant reports as required.
    • Collaboration with Funders, CHN Partners, & Lead Agencies- Partnering with ADAMH, behavioral health organizations, and lead agencies to identify consumers and coordinate access to rental subsidies and permanent housing. Participating in external meetings and maintaining positive relationships with case workers and agency representatives.
    • Leasing & Recertification Support - Collaborating with the Property Management Division to support residents through the leasing process and into housing placement and assisting with recertification activities as needed.

    Qualifications

    What you bring to the role:

    • Bachelor’s degree in social work, Psychology, Human Services, or related field.
    • 2 years’ experience in social services work
    • 40 hours/week availability
    • Valid driver’s license and auto insurance
    • Excellent written and verbal communication skills required with the ability to communicate across all levels both internally and externally.
    • Proficient in Microsoft Office Suite Products and ability to learn and adapt to changing technology utilized in role.
