    Position Description

    Under the direction of the Director of Housing, the DBH/Opioid Abatement Housing Case Manager provides strength-based, housing-focused case management services to high-risk populations impacted by opioid use, substance use disorders, mental health challenges, reentry, and other social barriers. This role supports clients in achieving housing stability, self-sufficiency, and overall wellness while ensuring compliance with program and funder requirements.

    Qualifications

    • Bachelor’s degree in Social Work, Criminal Justice, Behavioral Health, or related field required
    • Minimum 2–3 years of case management experience preferred
    • Experience working with high-risk populations, including individuals impacted by substance use, mental health disorders, reentry, or homelessness
    • Knowledge of housing-first principles and community-based resources
    • Familiarity with HMIS and electronic documentation systems (experience with Apricot preferred)
    • Strong written and verbal communication skills
    • Ability to manage multiple priorities and meet reporting deadlines
    • Ability to work independently and exercise sound judgment

    Responsibilities

    • Conduct client recruitment, screening, intake, and eligibility determination
    • Develop individualized service plans and monitor client progress
    • Provide housing-focused case management and crisis intervention
    • Support clients in overcoming barriers to permanent housing, employment, and stability
    • Conduct jail outreach to assess housing needs of potential participants
    • Complete assessments including biopsychosocial, self-sufficiency matrix, and SPDAT
    • Document all client interactions within required timeframes in Apricot and HMIS
    • Maintain accurate client files and program reports
    • Coordinate with community partners and social service agencies
    • Facilitate life skills groups and community meetings
    • Collaborate with program staff and resident monitors to ensure client well-being
    • Conduct routine drug screenings and document results
    • Maintain assigned housing site standards and tenant records
    • Attend required trainings, supervision, and staff meetings

    DESCRIPTION: The Case Manager provides specialized support to women returning home from prison. The case manager is responsible for recruiting perspective participants, providing admissions procedures, ensuring eligibility, developing service planning, documenting progress and needs, accessing and linking participants to resources, and discharge planning. The case manager will also be responsible for documenting case notes and program activities, entering data and prepare monthly reports reflecting such activity, as indicated. This position is under the supervision of the Program Manager.

    QUALIFICATIONS

    • Associate or Undergraduate degree in social work preferred or behavioral science and or Human Service

    • Minimum of 3-year successful experience in adult case management

    • Have direct experience working with homeless individuals of diverse backgrounds and disabilities including reentry, physical, mental, substance abuse, HIV/AIDS, etc.

    • Knowledge of reentry best practices, criminal justice system, court system and organizations, procedures, and terminology.

    • Outstanding organizational, verbal, and written communication skills

    • Ability to perform daily duties to achieve excepted outcomes and performance measures with minimal supervision

    RESPONSIBILITIES

    • Provide case management to clients as assigned. Case management will involve, screening, intake, assessment, service plan development, monitoring, linkage to appropriate community resources, follow up, appropriate discharge, tracking, etc.

    • Maintain up to date information regarding tenant occupancy, move outs, and vacancies.

    • Meet with Resident Monitor and other team members on a weekly basis.

    • Maintain thorough and complete record of participant's activity/progress.

    • Establish and maintain collaborative working relationships with housing organizations and other community resources.

    • Prepare and submit reports to supervisor on a timely manner basis.

    • Enter participant's information into the database in a timely matter and update as needed

    • Attend appropriate coalition and other community resource meetings.

    • Attend training and workshops as they come available,

    • Perform other assigned duties in a timely and efficient matter.
