CASE MANAGEMENT COORDINATOR

Job Summary:
We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing comprehensive support and guidance to individuals in need of behavioral health services. Your role will involve assessing patient needs, developing treatment plans, coordinating care, and advocating for the well-being of your clients. Duties and Responsibilities:
- Conduct patient interviews and assessments to determine their needs and develop individualized treatment plans
- Collaborate with healthcare, social service, and housing professionals to coordinate and monitor patient care
- Connect clients to addiction counseling and provide support to individuals struggling with substance abuse
- Deliver services that reflect our trauma-informed approach to care
- Assist clients with discharge planning and transition to appropriate levels of care
- Advocate for clients' rights and ensure they receive necessary services
- Maintain accurate and up-to-date documentation of client interactions and progress
- Collaborate with social workers, therapists, and other professionals to provide holistic care
Requirements:
- Bachelor's degree in social work, psychology, or related field (Master's preferred)
- Proven experience in case management or related field
- Knowledge of addiction counseling, behavioral health, and acute care services
- Excellent communication and interpersonal skills
- Ability to assess patient needs and develop appropriate treatment plans
- Proficient in documentation and record keeping
