Incident Management Specialist
Reporting Relationship: Incident Management Coordinator
Base Schedule: This position is full-time with on-call responsibility. The role is considered Non-Exempt under the Fair Labor Standards Act and is eligible for overtime pay.
Position Summary: The Incident Management Specialist assists the Incident Management Coordinator in overseeing the safety and welfare of the people we support, including allegations of neglect and abuse, financial exploitation, and errors in care.
Essential Job Functions:
- Investigate incidents and allegations of abuse in accordance with Parts 624 and 625, and complete all required documentation and follow-up requirements.
- Conduct investigations as needed regarding incidents relating to Corporate Compliance, Code of Conduct, and HIPAA laws and regulations.
- Support the Incident Management Coordinator with reporting, documentation, and training for investigations assigned to Opengate and investigations assigned to OPWDD and/or the Justice Center.
- Support the Incident Management Coordinator to input and monitor investigatory updates/uploads into the state incident management systems (IRMA< RIA< WSIR) and communicate them to the state agency per Parts 624 & 625 regulations as directed.
- Assist in facilitating the timely implementation of corrective action plans related to incidents and allegations.
- Gather data for input and trend analysis, and participate in compliance monitoring.
- Assist with preparing data for incident trends, reporting, and follow-up.
- Monitor to ensure Opengate's programs comply with agency policies and procedures.
- Monitor to ensure Opengate's programs comply with federal and state regulations and policies.
- Perform targeted audits as directed.
- Assist in the development of Plans of Corrections as directed.
- Monitor to ensure the interests of the supported people are being pursued and their rights are protected.
- Participate in committees as assigned.
- Demonstrate knowledge and commitment to the agency's mission, vision, and values.
- Follow federal, state, and local regulations to maintain a safe, healthy, and clean work environment, and report health and safety concerns to your supervisor.
- Identify and report compliance risks to the COO or CEO, as appropriate.
- Other duties as assigned.
Qualifications:
- High School diploma as required, associate's degree in human or social services preferred.
- Minimum of 2 years of compliance/quality improvement, utilization or program review.
- Must be an OPWDD-approved investigator with at least two years of experience conducting and completing Part 624 and Part 625 investigations.
- Knowledge of state and federal regulations as they pertain to healthcare providers and operators of OPWDD services and the New York State Justice Center.
- Minimum of 3 years of experience with OPWDD systems, including WSIR and IRMA.
- Strong written and verbal communication skills.
- Strong organizational and time management skills.
- Computer skills and capacity to navigate and enhance proficiency in software systems as needed. Working knowledge of Microsoft Office and similar programs.
Physical Requirements:
This position operates in a professional office environment. The role routinely uses standard office equipment and requires the ability to lift, push, and pull in order to perform general office duties (filing, etc.). This would require the ability to move files, open filing cabinets and bend or stand as necessary.
Compensation will be commensurate with job qualifications and work experience.
