At CHP, collaboration is not a buzz word, it's an active practice. We create a consistent space for idea generation and exploration, then offer connections, business services, and as appropriate, seek funding to bring to fruition the best ideas with potential for strengthening the health of the region. We lead, facilitate, connect, innovate and solve problems.
Currently, CHP has four priority programs that we support in the community:
(1) CHP supports a network of safety net clinics to offer donated medical services to low-income, uninsured people and provides patient navigation services and training to safety net providers;
(2) CHP provides planning, administrative, and data services for the Pikes Peak Continuum of Care to build an effective system of care for people experiencing homelessness;
(3) CHP provides support for a coalition of stakeholders engaged in community-centered planning to address the opioid epidemic; and
(4) CHP serves as the backbone organization for a county-wide collaborative aimed at reducing suicides by 20% by 2024.
