Minnesota Dignity Collaborative (MDC) is a grassroots nonprofit building community-driven solutions to housing instability and healthcare access.

We’re working toward a future where every Minnesotan has access to stable housing and reliable healthcare. Through volunteerism, partnership, and compassion, we strengthen the connections that help neighbors support one another and create a more resilient, caring community.

The Minnesota Dignity Collaborative (MDC) is a nonprofit organization dedicated to restoring dignity and stability for Minnesotans experiencing challenges with housing and healthcare. We believe that lasting change happens not only through systems, but through relationships — by neighbors walking alongside one another with empathy and purpose.

Our flagship initiative, Pathway to Home (PTH), brings this belief to life. Through PTH, trained community volunteers—called Navigators—partner with individuals and families who are struggling to find or maintain stable housing. Navigators help participants connect to local resources, healthcare, and supportive networks, but just as importantly, they offer understanding, encouragement, and a sense of belonging.

PTH was created from the recognition that navigating housing, healthcare, and community systems can be overwhelming, especially for those facing barriers or trauma. By combining compassionate relationships with practical support, we help people take steps toward long-term stability — and toward feeling at home again, in every sense of the word.

MDC also works to build bridges among local organizations, faith communities, and civic leaders, creating a shared ecosystem of care across Minnesota. Together, we’re cultivating a movement rooted in dignity, connection, and hope.
