Volunteer Referral Care Coordinator

Care Coordinator supports the delivery of integrated, patient-centered care by coordinating specialty referrals and follow-up care. This role serves as a liaison between patients, providers, and external partners to ensure continuity of care and reduce barriers to access.

Ensure accurate and timely documentation in the electronic medical record (EMR). Collaborate with providers when there are barriers to patients’ ability to access specialty care.

Maintain HIPAA and OSHA compliance. Support Medicaid Accountable Care strategies through accurate data entry and reporting
