Chart‑A‑Life volunteers help restore identity, dignity, and connection for individuals living with Alzheimer’s and dementia.
Volunteers create personalized memory collages using meaningful photos, words, symbols, and memories that reflect who a person was before the onset of the disease. These visual “life charts” create a walk down memory lane, offering comfort and familiarity to patients while helping caregivers, family, and friends better understand and connect with the whole person behind the diagnosis.
These collages often become powerful conversation starters—and lasting reminders of a life richly lived.
