    Descripción

    Case Management Follow-Up

    The Case Management Follow-Up Assistant plays a vital role in supporting the well-being of our clients by conducting follow-up communications to ensure individuals and families are successfully connected to the services and referrals provided. This volunteer helps track progress, identify unmet needs, strengthen relationships with those we serve, and measures organization’s impact.

    Key Responsibilities:

    • Make follow-up phone calls, text messages, or emails to clients to check on the status of referrals, housing applications and support services (e.g., food, transportation, healthcare).
    • Assist with updating client records and notes in our case management system.
    • Confirm housing conditions (e.g., homeless, housing fulfilled).
    • Escalate urgent issues to case managers when necessary.
    • Maintain accurate, timely, and confidential documentation.
    • Provide empathetic, respectful, and non-judgmental communication.
    • Participate in brief volunteer check-ins or team meetings as needed.

    Qualifications:

    • Must have a bachelor’s degree in social work, psychology, human services, or a related field or be currently enrolled in a degree program in a related field.
    • Excellent communication and listening skills.
    • Dependable and organized with good attention to detail.
    • Comfortable using phone, email, and basic database systems (training provided).
    • Ability to maintain client confidentiality and follow ethical guidelines.
    • Patience and compassion for individuals in crisis or facing barriers.
    • Bilingual (Spanish/English) a plus but not required.

    Training & Support Provided:

    • Orientation to My New House’s mission, services, and policies.
    • Training on follow-up procedures and case documentation.
    • Ongoing support and supervision from case management staff.

    Training Provided:

    All volunteers will receive orientation and ongoing support. Basic training in Medi-Cal billing procedures and our data tracking system will be provided.

    Impact:

    Your service will directly support our ability to sustain housing services for individuals and families in need by ensuring we can recover eligible reimbursements and evaluate our program's effectiveness.

    My New House is a 100% volunteer-based organization. We are not hiring for paid positions at this time. Instead, we welcome individuals who are passionate about learning and creating meaningful impact in underserved communities. Volunteers play an essential role in sustaining our programs and services.
