I am excited you are interested in volunteering with Traditions Health. Volunteering with hospice is such a rewarding experience. I am thrilled you want to be part of our volunteer team. There are many opportunities available. We are in partnership with assisted living/nursing homes/ group homes here in town, as well as in surrounding areas, and participated in many fun activities with the residents. However, our biggest need is sitting with patients in their homes so the caregiver can have some time away. You can volunteer as much or as little as your schedule permits. We will work around your other commitments.

Traditions Health provides a training/orientation to assure that your prepared and comfortable to meet your volunteer assignment.

Attached you will find the volunteer application as well as two reference forms. You will need to have TWO references. The reference form is a little confusing. Please follow the directions below to ensure they are filled out correctly.

• Fill out the top section with your reference’s contact. The middle section is for your information, the position/relationship to the person you are asking to be a reference, and how long you have known that person.
• Make sure you sign and date this middle sections. YOU DO NOT HAVE TO PUT YOUR SOCIAL SECURITY NUMBER ON THIS FORM. The bottom portion is the part that your reference will fill out. you can return the form to me (with the top two sections complete) and I will call the person.
• PLEASE NOTE: You can have personal references
