    Human Health Project (HHP) is a holistic health literacy charity. Our mission is to improve the health of the underserved and vulnerable through holistic information, education and advocacy.     This role centers on your ability to foster strong relationships between our non-profit organization and the community we serve. Specifically, you'll be instrumental in establishing and managing a referral network to connect individuals with the services they need.

    Job Overview

    In this position, you'll build and maintain relationships with community members, leaders, other organizations, and stakeholders to promote our organization's mission and services. Your key responsibilities will include understanding community needs, facilitating effective communication, and connecting individuals to resources through a robust referral network.

    Duties and Responsibilities

    Your responsibilities will include:

    • Building strong relationships with diverse community members and organizations.
    • Acting as the primary point of contact for community inquiries.
    • Developing and implementing effective outreach strategies.
    • Attending community events to gather valuable feedback and share information about our work.
    • Facilitating clear and consistent communication between our organization and the community.
    • Identifying specific community needs to inform and guide our program development.
    • Establishing and managing a comprehensive referral network to connect individuals seamlessly with needed services.
    • Providing accurate information and direct referrals to appropriate services.
    • Collaborating effectively with various internal departments to ensure cohesive support.
    • Maintaining meticulous records of all interactions and referrals.
    • Reporting regularly on engagement and referral activities, highlighting successes and areas for improvement.
    • Potentially mediating conflicts that may arise within the community or referral process.

    Qualifications

    Typically, you'll need a Bachelor's degree in a related field or equivalent relevant experience. Experience working with diverse communities, along with strong communication and interpersonal skills, are crucial. Familiarity with local community resources and dynamics will be highly beneficial. Basic computer proficiency is required, and a valid driver's license may be necessary for community travel.

    Essential skills for this role include strong communication, relationship management, networking, and active listening. Your ability to collaborate, your organizational skills, time management, problem-solving, and data analysis abilities are also highly valued. Furthermore, cultural competence and conflict resolution abilities are important for success in this position.

    Preferred Qualifications

    • Bachelor’s degree in Social Work, Community Development, Public Health, or a related field (or equivalent relevant experience).
    • 2+ years of experience working with community-based organizations or grassroots initiatives.
    • Familiarity with local community resources, service providers, and networks.
    • Strong facilitation and public engagement skills.
    • Valid driver’s license for local travel (if required).

    Time Commitment

    • This is a part-time volunteer role requiring 15–20 hours per month.
    • Hours are flexible and can be adjusted to fit the volunteer’s availability.
    • Occasional evenings or weekends may be required for community events or urgent referral needs.

    Remote/In-Person Status

    • Hybrid role: Primarily community-based/in-person to foster direct relationships.
    • Some responsibilities, such as reporting, record-keeping, and virtual check-ins, can be completed remotely.

    • Chief Business Development Officer (CBDO) & Chief Executive Officer (CEO)
