Our philosophy is to empower our patients and families by providing the medical and social support necessary to embrace life. We focus on the practical side of living. Our job is to assist our patients and families through the physical, emotional, and spiritual challenges of their health needs. To our patients, "It's about living"!

Transitions Care offers end of life care, symptom management and primary care to all who are in need of our services. To us, each patient is unique- we tailor our plan of care to each individual's needs. That's why we serve our patients wherever they reside, in a variety of settings including: Home or private residences, independent living, assisted living and skilled nursing facilities, senior or retirement communities, acute care facilities, outpatient clinics, and hospitals. Transitions Care is serving the Pittsburgh area in the counties of: Allegheny, Beaver, Butler, Fayette, Lawrence, Washington, and Westmoreland.
