Heritage OnCare provides Hospice care wherever the patient calls home - private residence, nursing home or assisted living community. In collaboration with a patient's physician & Hospice Medical Director, we focus on quality of life using Expert medical care, pain & symptoms management, and emotional & spiritual support.

Heritage Communities was founded by Farhan Khan, CEO & Nate Underwood President, with the first community, Shalimar Gardens opening in Fremont Nebraska in 2000. Over the past 20+ years we have grown to more than a dozen communities in four different states with over 1,200 associates and volunteers all working toward the same mission of Living Better and upholding the same core values of trust, respect, compassion &fun.
