One Community Hospice

One Community Hospice & Palliative Care is a company that strives to support every patient and their family. We have a great team that communicates well and is always working towards making sure that every patient feels like a person and not a number. Our volunteers provide companionship in homes as well as Nursing home facilities. We are a Level 4 in We Honor Vets program and working towards our Level 5. Volunteers also help with care giver relief where they stay with the patient while the caregiver takes a break or goes to an appointment. Volunteers also train to do Increased Presence where the sit with a patient and/or their family during their final hours. We are looking to expand our volunteer program to include other services for our home patients.
