    MAC Angels Foundation is currently hiring a compassionate and dedicated licensed clinical social worker (LMSW) licensed to practice and registered in New Jersey and New York State to join our multidisciplinary team serving individuals with Amyotrophic Lateral Sclerosis (ALS).

    MAC Angels Foundation is a 501(c)(3) nonprofit organization based in Westchester, New York serving the Greater New York City Metropolitan Area. The Foundation provides support, guidance, and resources critical to quality of life and care for individuals and their families living with ALS.

    The goal of the organization is to provide unique, non-medical services tailored to the individual needs of people with ALS (pALS), family, and caregivers. Services include patient advocacy, coordinating care, case management and mental health services.

    Position Description:

    The social worker will serve in a key role in providing the highest quality of support and care to pALS and their families. As a valued member of the team, you will commit to fostering an environment that builds creative thinking, problem solving, and empowerment. Your contribution encourages a respectful and dignified environment for our multidisciplinary team, pALS, families, and caregivers.

    This position is remote, although when appropriate, the responsibilities will include field-based visits to pALS. Attendance at monthly staff meetings is required. This position will report to the Executive Director and will have the following responsibilities:

    • Connect pALS and their families to community resources and play an important role in coordinating care.
    • Provide guidance on insurance and disability issues to pALS and their families.
    • Assist with legal paperwork related to patients’ medical care (i.e. power of attorneys, directives, etc.)
    • Conduct home visits as needed, when appropriate, to provide psychosocial assessments.
    • Address family or personal relationship challenges that arise and work to develop family resilience.
    • Provide general counseling.
    • Responsible for compliance with all regulations, laws, policies and procedures applicable to providing consultative and supportive home visits and protecting patient privacy.
    • Ensure all client interventions are properly documented and logged.

    Position Qualifications:

    • Social worker (LMSW) licensed to practice and registered in New Jersey and New York State.
    • Ability to work as part of a team and contribute to team meetings.
    • Hospice or palliative care experience.
    • A strong work ethic and personal integrity.
    • Ability to demonstrate organizational and communication skills which would enable effective interaction with other staff, pALS, families, physicians, health care providers, and the community.
    • Experience addressing needs of pALS, families, or working with the ALS community is helpful
    • Knowledge of Medicaid rules, regulations, loopholes, and procedures helpful.
    • Driver's license with clean, acceptable driving record and the ability to transport self to necessary offices, facilities, and private residences.
    • Comprehensive knowledge of community resources.
    • Knowledge of case management best practices is helpful.

    This is a part-time position (per diem) with variable hours depending upon the needs of pALS and caregivers. We offer a competitive rate.
