    The STAR Health Center at SUNY Downstate Health Sciences University is a comprehensive ambulatory care program caring for the most vulnerable populations in Central Brooklyn, including people with HIV and HCV, immigrants, LGBTQ+ populations, reentrants, and people with mental health and substance use disorders. We seek talented, dedicated individuals from all walks of life who possess a strong commitment to our mission.

    STAR's Supportive Counseling Project is seeking a Patient Navigator to provide field-based health education on a monthly or weekly basis and skills building service to patients. Patient Navigators at Brooklyn’s STAR Health Center are essential members of our interdisciplinary team and help individuals navigate the healthcare and public benefits systems. This is a great opportunity for social justice advocates or gap year students who desire a position that addresses the social determinants of health through provision of care coordination services to people living with HIV. Patient Navigators address barriers to care such as food insecurity, LGBTQ+ stigma, homelessness, substance use dependency and/or mental health issues by conducting home visits, arranging transportation, accompanying patients to appointments, and providing health promotion and adherence support.

    Requirements: A.A. or A.S. degree with at least 2 years of behavioral health case management experience working in a community-based agency or health care organization. Effective computer, verbal, organizational and interpersonal skills required. Must be computer literate with a proficiency in word processing.

    Preferred experience: At minimum 1 year of counseling/health promotion experience in a community-based agency or health care organization preferred. Bilingual Creole or Spanish speaking is an asset. Outreach and community-based experience.

    Salary Range: $50,000 - $51,500 based on education and experience with comprehensive benefits.
