The STAR Health Center at SUNY Downstate Health Sciences University is a comprehensive ambulatory care program caring for the most vulnerable populations in Central Brooklyn, including people with HIV and HCV, immigrants, LGBTQ+ populations, reentrants, and people with mental health and substance use disorders. STAR's Supportive Counseling Project is seeking a Patient Navigator to provide field-based health education on a monthly or weekly basis and skills building service to patients.
