    Position: NY Connects Education and Outreach Coordinator

    Basic Function:

    The NY Connects Program provides Information and Assistance and Options Counseling/Person-Centered Counseling on Long-Term Services and Supports (LTSS) for older adults and people with disabilities.

    The Education and Outreach Coordinator is responsible for developing and overseeing the program’s outreach and education initiatives, building and maintaining community partnerships, supervising outreach staff, and ensuring compliance with New York State program standards. This position requires experience in community engagement, public speaking, event coordination, staff supervision, and program planning. The role requires travel throughout New York City and strong organizational, communication, and time-management skills. Prior management experience is strongly preferred.

    Specific Duties:

    • Plan and conduct in-person, off-site, and virtual workshops, presentations, and outreach events to educate consumers, community partners, and other stakeholders about Long-Term Services and Supports (LTSS), public benefits and available community resources.
    • Develop and maintain organizational tracking systems and Excel spreadsheets to monitor outreach activities, performance metrics, and contractual deliverables, and prepare timely reports for the Program Director.
    • Ensure compliance with New York State NY Connects Program Standards related to outreach and education activities.
    • Collaborate with the NY Connects Program Director to evaluate outreach performance, identify barriers, and develop strategies to achieve program goals and contractual requirements.
    • Build and maintain partnerships with community-based organizations, government agencies, healthcare providers, hospital social workers, home and community-based service providers, coalitions, and other stakeholders to strengthen referral networks and ensure accurate information and resource sharing.
    • Cultivate cross-referral relationships with organizations serving older adults and people with disabilities.
    • Assist in developing, reviewing, and updating outreach, educational, and training materials.
    • Maintain calendars for workshops, trainings, outreach events, and community presentations.
    • Supervise and support a team of approximately 4-5 Outreach Specialists by providing guidance, oversight, and assistance in planning and implementing outreach activities.
    • Coordinate onboarding and ongoing training for outreach staff, contribute to training curriculum development, and collaborate with borough managers to identify and address staff training needs.
    • Participate as a member of the NY Connects management team in developing and implementing policies, procedures, work plans, and quality improvement initiatives to ensure compliance with contract requirements and program goals.
    • Contribute to the NY Connects Resource Directory by identifying and incorporating community resources established through outreach activities and partnerships.
    • Order, organize, and maintain outreach materials and promotional supplies, ensuring adequate inventory levels.
    • Travel throughout the five boroughs of New York City to attend outreach events, meetings, trainings, and other program activities.
    • Attend occasional evening and weekend outreach events as needed.
    • Participate in agency-wide trainings, staff meetings, and other organizational activities.
    • Perform other duties as assigned.

    Education and Training:

    • Bachelor’s Degree required.
    • Minimum of three (3) years of supervisory experience.

    Experience:

    • Excellent oral and written communication skills.
    • Prior management experience strongly preferred.
    • Proficiency in Microsoft Office applications and internet research.
    • Ability to learn new software applications and database systems quickly.
    • Strong interpersonal skills with the ability to interact professionally with consumers, colleagues, community partners, and other stakeholders.
    • Ability to work independently and collaboratively.
    • Comfortable working respectfully with individuals from diverse backgrounds.
    • Strong attention to detail with the ability to manage multiple priorities effectively.
    • Experience providing workshops and conducting community outreach.
    • Knowledge of Long-Term Services and Supports (LTSS) and public benefits.
    • Understanding of, or experience working with, people with disabilities and disability-related issues.
    • Ability to travel throughout all five boroughs of New York City.
    • Bilingual skills preferred.

    Travel Demands:

    The position is based in CIDNY’s Manhattan office and currently follows a hybrid schedule of three days in the office and two days working remotely or in the field. Regular travel throughout New York City is required to attend outreach events, meetings, trainings, and other program activities.

    Salary:

    Up to $66,000 annually
