    Job Title: Program Community Manager

    Organization: Diabetes Foundation

    Reports To: Associate Program Director

    Since 1990, the Diabetes Foundation has been dedicated to improving the lives of people at risk for and living with diabetes through education, support, and access to essential resources. We empower individuals and families to prevent and manage diabetes through emergency assistance, self-management education, nutrition programs, community outreach, and partnerships with healthcare organizations, helping build healthier communities for generations to come.

    Position Summary

    As the Program Community Manager, you will provide in-person community outreach and telephone support while connecting participants with Diabetes Foundation programs and community resources. You will also build relationships with community partners, assist with educational events, coordinate volunteers, and collect program data to support reporting and grant requirements.

    Responsibilities

    • Provide in-person community outreach, education, and participant support.
    • Connect individuals with A1C screenings, diabetes education, healthcare appointments, community resources, and Diabetes Foundation programs.
    • Develop and maintain relationships with healthcare providers, community organizations, food pantries, senior centers, schools, and other partners.
    • Coordinate and support volunteers during community events, health fairs, and outreach activities.
    • Assist with workshops, educational presentations, and diabetes awareness events.
    • Schedule appointments, conduct participant follow-up, and maintain accurate records.
    • Collect, organize, and track program data to support grant reporting and program evaluation.
    • Communicate regularly with community partners to obtain required information and ensure accurate reporting.
    • Represent the Diabetes Foundation professionally within the community.
    • Perform other duties as assigned.

    Qualifications Required

    • Bilingual (English/Spanish) required – must be able to effectively communicate, speak, and provide support in both English and Spanish.
    • Valid driver's license and reliable transportation.
    • Excellent communication and interpersonal skills.
    • Strong organizational skills with exceptional attention to detail.
    • Proficiency in Microsoft Office (Word, Excel, Outlook, PowerPoint), Canva, email, and data entry.
    • Ability to work independently and collaboratively in a team environment.
    • Commitment to providing compassionate, respectful service to diverse communities.

    Preferred

    • Experience in community outreach, customer service, healthcare, nonprofit organizations, or social services.
    • Knowledge of diabetes, chronic disease management, or community health resources.
    • Experience coordinating volunteers or community events.

    Work Schedule

    • Full-time, hourly position
    • 40 hours per week
    • Monday–Friday, 9:00 a.m. – 5:00 p.m.
    • Occasional flexibility may be requested for community events, outreach activities, or special programs.
