Community Manager
Organization: Diabetes Foundation
Location: New Jersey
Reports To: Associate Program Director
About the Diabetes Foundation
The Diabetes Foundation is dedicated to improving the lives of individuals at risk for and living with diabetes through education, support, advocacy, and access to essential health resources. We work alongside community partners to provide programs and services that help individuals prevent and manage diabetes while building healthier communities.
Position Summary
The Community Manager serves as a direct connection between community members and the Diabetes Foundation's programs and services. This position provides community outreach, conducts A1C screenings, connects individuals to Pathways to Care services, diabetes education programs, healthcare appointments, and community resources. The Community Manager builds relationships with community partners, supports events and health fairs, collects program data, and helps ensure individuals receive the resources and support they need to improve their health and well-being.
Key Responsibilities
- Conduct community outreach and provide education and support to participants.
- Conduct A1C screenings at community events, health fairs, and partner locations.
- Connect participants to Pathways to Care services, diabetes education programs, healthcare providers, and community resources.
- Build and maintain relationships with healthcare providers, community organizations, food pantries, senior centers, schools, and other community partners.
- Coordinate and support volunteers during community events, health fairs, and outreach activities.
- Assist with workshops, presentations, and diabetes prevention and management programs.
- Follow up with participants and maintain accurate records of services provided.
- Collect, organize, and track program data to support reporting and grant requirements.
- Communicate regularly with community partners to gather information and ensure accurate program reporting.
- Represent the Diabetes Foundation professionally in the community.
Qualifications
- Bilingual English/Spanish required.
- Valid driver's license and reliable transportation.
- Strong communication and interpersonal skills.
- Excellent organizational skills and attention to detail.
- Proficiency in Microsoft Office (Word, Excel, Outlook, PowerPoint), Canva, email, and data entry systems.
- Ability to work independently and collaboratively in a team environment.
- Experience in community outreach, healthcare, nonprofit organizations, social services, or customer service preferred.
- Passion for serving diverse communities and improving health outcomes.
Schedule
- Full-time
- Monday through Friday, 9:00 a.m. to 5:00 p.m.
- Occasional evenings or weekends may be required for community events and outreach activities.
