Responsibilities
- Work closely with Quality Assurance RN Manager to assist members with newly diagnosed diabetes (DM) and members with known DM and high A1Cs and gaps in care.
- Telephonically contact members and providers to develop management plan, that best fits member’s needs.
- Provide diabetes education and support in a manner accommodating of the member's lifestyle, beliefs, and culture.
- Enable the member, in collaboration with her/his physician, to better understand the management of blood glucose, and develop coping skills to address the daily challenges of the disease
- Assist members in identifying patterns and opportunities for improvement in daily self-management, including making healthier food choices, staying physically active, monitoring blood glucose levels, taking medication as prescribed, and coping with the lifestyle changes that diabetes requires. Reduce the risk of complications, and potentially decrease the need of medications and eliminate the need for emergency room visits.
- Educate members to better understand how to use prescribed devices, such as meters, insulin pens, and continuous glucose monitoring devices.
- Perform additional duties and projects, as assigned by management.
Qualifications
- Must have a valid New York State Registered Nurse (RN) license required; Bachelor’s degree in Nursing preferred; and Diabetes Care and Education Specialist /Certified Diabetes Educator certification required.
- Minimum of three (3) years of Diabetes Care and Education Specialist (formerly referred to as Certified Diabetes Educator) experience and two (2) years nursing experience in ambulatory care, community health care or medical/surgical required
- Must have strong experience in clinical assessment, patient education, motivational interviewing, critical thinking, independent decision-making, quality assurance and quality improvement processes.
- Must have excellent nurse-physician communications skills.
- Intermediate computer and digital literacy
- Basic skills in Microsoft Outlook, Word and Excel preferred.
- Bilingual in Spanish preferred.
- Valid NYS Driver’s license with ability to travel in New York Metropolitan area preferred
- Ability to work flexible hours including evenings and weekends as needed
