    Patient Coordinator

    Part time, remote - New York based

    Location: Remote (must reside in New York State)

    Schedule: Part-time, approximately 20-25 hours per week

    Compensation: $25 per hour

    Reports to: Program Director

    Travel: Occasional travel within New York for meetings, conferences, and outreach events

    About End of Life Choices New York

    End of Life Choices New York (EOLCNY) is a nonprofit organization dedicated to expanding end-of-life options and improving end-of-life care, so all New Yorkers can experience the best possible quality of life at the end of life.

    We advocate for stronger rights for individuals facing serious illness, provide free support for advance care planning and caregiving, and offer education to the public on topics including advance directives, hospice, palliative care, patient rights, and medical aid in dying (MAiD).

    Many New Yorkers receive end-of-life care that does not align with their wishes and values, and only about 30% of adults have completed an advance directive. These challenges disproportionately affect communities that have historically experienced inequities in healthcare, including communities of color. Through education, advocacy, and patient support, EOLCNY works to ensure every New Yorker has access to informed, compassionate, and patient-centered end-of-life care.

    In 2025, New York passed the Medical Aid in Dying Act, expanding end-of-life options for terminally ill adults. EOLCNY has played a leading role in supporting implementation of the law across New York. As the law takes effect in August 2026, we are expanding our Aid in Dying Program to ensure patients, families, and healthcare professionals have access to the information, referrals, and the support they need.

    Position Summary

    The Patient Coordinator serves as the primary point of contact for individuals and families seeking information about MAID in New York. The Patient Coordinator will provide compassionate patient navigation, coordinate referrals to participating providers, maintain program data, and support implementation of EOLCNY’s Aid in Dying Program across the state.

    This position reports to the Program Director. While primarily remote, occasional travel within New York is expected for meetings, conferences, educational events, and outreach activities.

    Key Responsibilities:

    Patient Navigation

    • Serve as the first point of contact for patients, families, caregivers, and healthcare professionals seeking information about MAID.
    • Conduct intake calls and assess patient needs using a detailed intake form.
    • Coordinate MAID patient navigation, care team navigation, patient and family support, and additional resource referrals.
    • Provide confidential referrals to participating healthcare providers and other community resources.
    • Monitor patient progress and conduct appropriate follow-up throughout the navigation process.
    • Respond to phone and email inquiries in a timely, compassionate, and professional manner.
    • Identify barriers to care and help connect patients with appropriate resources.
    • Escalate complex patient situations to the Program Director as appropriate.

    Program Administration

    • Maintain accurate and confidential records of patients, providers, and services.
    • Manage databases and referral tracking systems.
    • Maintain organized workflows for patient intake, referrals, and follow-up.
    • Prepare monthly reports summarizing program activities, trends, and goals.
    • Provide administrative support for the Aid in Dying Program as needed.

    Program Evaluation and Outreach

    • Support educational outreach efforts, including conferences, presentations, tabling events, and community engagements.
    • Assist with emails communications and social media content related to the program.
    • Collaborate with the EOLCNY team to develop outreach plans, strategic goals, and program budgets.

    Preferred Qualifications:

    • One or more years of experience in patient navigation, medical navigation, healthcare coordination, hospice, palliative care, social work, counseling, or another client-facing healthcare role.
    • Bachelor’s degree in education, social work, counseling, public health, healthcare administration, nonprofit management, or a related field (or equivalent relevant experience). Master’s degree preferred.
    • Experience supporting individuals and families experiencing serious illness, grief, or end-of-life planning is highly desirable.

    Required Skills:

    • Excellent written, verbal, and interpersonal communications skills.
    • Demonstrated ability to communicate with empathy, professionalism, and cultural humility during emotionally sensitive conversations.
    • Strong organizational and time-management skills with the ability to manage multiple priorities simultaneously.
    • Exceptional attention to detail and commitment to maintaining accurate, confidential records.
    • Proficiency with Microsoft Office (particularly Powerpoint, Word, and Excel), Zoom, and the ability to quickly learn Bloomerang (CRM system).
    • A sincere commitment to EOLCNY’s mission and to advancing compassionate, patient-centered end-of-life care.
    • Ability and willingness to travel periodically within New York State.

    Additional Details:

    • Flexible weekday schedule based on program needs.
    • This position is funded through a one-year grant with the possibility of renewal based on future funding.

    Interview Process:

    1. A one-hour interview with the hiring manager either in-person or via Zoom.
    2. A one-hour interview with the hiring manager and the Executive Director, Mandi Zucker, either in-person or via Zoom.
    3. If the interviews are conducted virtually, finalists may be invited to participate in a brief in-person meeting with the hiring manager.
