Fee-for-Service Nurse

Concerned Home Managers for the Elderly (COHME), Inc. is a licensed home care agency that has been serving older adults since 1984. We provide home health aides to elders in their homes, with a mission focused on providing excellent care to seniors while supporting the home health aides who care for them.

COHME, Inc. is currently seeking a Nurse to provide nursing services and conduct home visits mostly in Manhattan with some visits in Brooklyn or Queens.

Compensation: $125 per home visit.

Responsibilities:

• Provide assessment of health needs and eligibility for home care through intake visits.
• Reassess client needs through revisits and recertifications.
• Perform aide supervision and orientation for home health aides who are new to their cases.
• Develop and implement treatment plans in conjunction with the authorized medical provider, including communication regarding changes in client condition.
• Prepare electronic documentation and summaries of client progress as needed for the continuation of home health services.
• Ensure compliance with New York State Department of Health requirements and standards for licensed home care agencies.
• Work closely with social work, administrative, and field staff.
