    • Our Education & Career Services program imparts individual community members with the practical tools necessary to build secure and prosperous futures.

    Job Title: Community Health Worker

    FLSA: Non-Exempt

    Model: In person

    Summary: Community Health Workers, who work within the community and in health care settings, empower patients and their caregivers to improve health outcomes by providing culturally sensitive education, peer-based support, and connections to primary care and community-based social supports. This position is with NMIC but works in close partnership with New York-Presbyterian (NYP) Hospital and reports to two managers, one at NMIC and one at NYP.

    ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

    • Enrolls and manage caseload of patients who meet risk program criteria
    • Helps patients set and achieve program goals
    • Supports patients to navigate the health care system
    • Connects patients to social resources based on identified needs
    • Helps patients enroll onto/navigate patient portal
    • Conducts at least 2 home visits and provides appointment accompaniment as needed
    • Delivers practice-based education support (101)
    • Offer practice-based support and information sessions at an assigned patient centered medical home, CBO or hospital. Collaborate with the healthcare team throughout the patient centered medical home and hospital.
    • Complete appropriate documentation of assessments and educational sessions using required data systems.
    • Submits all required documentation by designated deadline
    • Maintain an organized filing system and submit all paperwork and documentation to program staff by designated deadlines.
    • Facilitate/host at least two community-based workshops per year.
    • Conduct community-wide outreach and assist with event planning, including workshops and health fairs.
    • Make social services and other referrals to address social determinants of health.
    • Attend all required staff meetings and trainings at both NYP and NMIC.

    Additional related responsibilities may be assigned.

    QUALIFICATIONS:

    • 2+ years of CHW experience or comparable education/training and experience strongly preferred.
    • Experience facilitating workshops and providing one-on-one health support and information is preferred.
    • Willingness to visit with families in the hospital and in the community (comfortable conducting home visits).
    • Strong interpersonal skills demonstrative of empathy and support.
    • Strong written and verbal communication.
    • Demonstrated ability to work collaboratively.
    • Demonstrated ability to work with families who face exceptional challenges.
    • Solid Microsoft Office skills.
    • Bilingual (English/Spanish) required.

    Please note this position follows NYP’s onsite vaccination requirements.

    SALARY/BENEFITS: NMIC offers a competitive salary based on experience and education as well as a comprehensive and generous benefits package: medical, dental, life, and disability insurance are included; paid time off, including 20 vacation days, 15 sick days, 5 personal days, and 11 agency holidays; plus the week of Christmas Eve through New Year’s Day; a 401k with agency contribution; and other benefits.

    SALARY: The salary for this position is $55,000 per year.
