    Descripción

    WeAchieve, Inc. is seeking a Person Centered Planning (PCP) Specialist to join our Quality Assurance & Planning team. This highly field-based role supports the development, coordination, implementation, and ongoing quality review of Person Centered Plans for people receiving services from WeAchieve.

    The PCP specialist works directly with people supported, Direct Support Professionals (DSPs), families, program teams, and other stakeholders to ensure each person's voice, preferences, goals, and desired outcomes are meaningfully reflected in their plan and in the services they receive.

    Key Responsibilities:

    • Facilitate person-centered pre-planning activities with people supported in home, program, and community settings.
    • Ensure each person's voice, strengths, preferences, interests, goals, and desired outcomes are meaningfully reflected throughout the planning process.
    • Coordinate PCP activities from pre-planning through finalization and implementation, including required documentation, signatures, and follow-up.
    • Conduct field observations to assess whether services and supports are being delivered consistently with each person's PCP.
    • Identify gaps between the written PCP and actual service delivery and coordinate appropriate follow-up with DSP's, supervisors, and program leadership.
    • Review PCP-related documentation for completeness, accuracy, timeliness, and alignment with DDA and organizational requirements.
    • Track progress toward individual goals and desired outcomes and follow up on identified documentation or implementation concerns.
    • Conduct annual PCP training for staff on approved Person-Centered Plans, including individual goals, support strategies, and implementation concerns.
    • Provide coaching and technical assistance to DSPs and program staff regarding person-centered practices and PCP implementation.
    • Collaborate with families, authorized representatives, Coordinator of Community Services, program teams, and other members of the person's support team.

    Requirements:

    • Bachelor's degree in human services, Social Work, Psychology, or a related field preferred; equivalent relevant experience in DDA-funded or developmental disability services may be considered.
    • Experience supporting people with disabilities and/or working with Maryland DDA-funded services strongly preferred.
    • Knowledge of person-centered planning principles and Maryland DDA service delivery requirement.
    • Familiarity with Maryland DDA PCP documentation and processes; experience with LTSSMaryland and ICM preferred.
    • Strong organizational, time-management, documentation, communication, and follow-through skills.
    • Ability to independently manage multiple PCP timelines, field appointments, competing priorities, and follow-up activities.
    • Ability to build effective working relationships with people supported, families, DSPs. program staff, and external partners
    • Valid driver's license and reliable transportation required.
    • Ability to travel regularly throughout Montgomery County and designated areas of Prince George's County.
