Transition Coordinator - Hyde Park Day School

    Part-Time (32 Hours) | Chicago Campus

At Hyde Park Day School (HPDS), we believe every child with a diagnosed learning disability deserves the opportunity to thrive academically, socially, and emotionally. Our specialized programs help bright students build the skills and confidence they need to succeed in their next educational environment. We are currently seeking a dedicated and experienced Transition Coordinator to join our Chicago campus team.

    About the Role

The Transition Coordinator plays a vital role in supporting students as they move from HPDS to schools in their home communities. This position involves collaborating with families, educators, and service providers to ensure each student's transition is as seamless and successful as possible. You will assess individual student needs, identify appropriate school placements, coordinate necessary supports, and provide follow-up services after students have transitioned. In addition, you will plan and manage graduation activities, from ceremonies to yearbook production, helping our students and families celebrate their achievements.

    Key Responsibilities

While every day will bring new opportunities to make a difference, your work will include:

    • Coordinating educational placements for students transitioning from HPDS
    • Meeting with parents, teachers, and service providers to ensure alignment with each student's learning needs
    • Leading and supporting the IEP process for transitioning students
    • Organizing and overseeing graduation events and related activities
    • Maintaining ongoing professional development and sharing best practices with colleagues
    • Representing HPDS at meetings with partner schools and in the broader educational community

    Qualifications

    The ideal candidate will bring:

    • A bachelor's degree or higher in learning disabilities, special education, or social work (Master's preferred)
    • Proven experience placing students with learning disabilities in appropriate educational settings
    • Knowledge of public and private schools within the Chicago metropolitan area
    • At least three years' experience working with children with diagnosed learning disabilities (preferred)
    • Strong organizational, problem-solving, and communication skills
    • The ability to work effectively and efficiently while managing multiple priorities
    • A warm, approachable demeanor that fosters trust with students, parents, and colleagues

    Why Work With Us

Joining HPDS means becoming part of a mission-driven, collaborative community of educators and professionals committed to helping students realize their full potential. You will have the opportunity to:

    • Make a direct, lasting impact on students' educational journeys
    • Work in a supportive, collegial environment that values your expertise
    • Access professional growth opportunities to continue developing your skills
    • Receive a competitive salary and comprehensive benefits package, including medical, dental, vision, retirement plans, paid time off, and tuition reimbursement

If you are passionate about supporting students with learning disabilities and thrive in a role that blends educational expertise with compassionate guidance, we encourage you to apply.

Apply today and help shape the next chapter in our students' stories.
