Job Summary:
The Housing Case Manager will support victims of domestic violence with identifying and applying for, safe, secure, and affordable permanent housing, will help clients negotiate rental agreements with landlords, provide case management to promote housing retention and provide support and follow-up assistance to program participants.
Duties and Responsibilities:
- The Housing Case Manager will work as part of an interdisciplinary care team;
- Assist clients to help them acquire permanent housing;
- Provide crisis intervention, basic services, short-term and long-term case management services to survivors of domestic violence;
- Develop effective, trusting relationships with clients with a focus on facilitating their move towards independence;
- Perform a comprehensive needs assessment (screening and intake) for all case-managed clients, and in conjunction with a client, develop a plan that is based on the client’s needs, issues and goals;
- Provide information, referrals and social services advocacy to the client, as needed;
- Maintain accurate documentation of client information, service delivery, goal plan, progress notes, etc.;
- Network with agency and outside service providers, ensuring client access to appropriate resources;
- Educate community members, including potential clients, about community services;
- Provide life skills counseling as appropriate;
- Complete appropriate paperwork in an efficient and timely manner;
- Works with Project Manager, Counselors and other staff to ensure goals of the clients are met;
- Provide follow – up visits to clients after placement in permanent housing;
- Attend training and other staff meetings as required;
- Other miscellaneous duties as assigned.
Requirements:
Must support Jenesse's mission and philosophy and be able to function within a multi-cultural, multi-ethnic crisis center setting as part of a multi-disciplinary team. Must have good communication skills.Must be drug-free, non-alcohol abusing and able to work in a smoke free environment.Ability to attend and complete 40 Hour Domestic Violence Certification course.
Experience and Education:
Must have a high school diploma. Experience locating housing, working with landlords and supportive housing agencies preferred. Experience working with people in crisis preferred.
Salary Range:
$55,000.00 - $75,000.00
