Haley House uses food and the power of community to break down barriers, open opportunities, and strengthen the neighborhoods we serve. Founded in 1966, Haley House has grown to include five interconnected branches: direct services (soup kitchen, food pantry, meal for elders), permanent affordable housing, urban agriculture, education and training (Take Back the Kitchen and LiFT: Re-Entry Program), and social enterprise. Our direct services are located in the South End and Roxbury.
Role:
The Case Manager will identify service needs and facilitate access to services relating to mental and physical health, substance use disorders, and accessing and maintaining stable employment and housing for individuals that are within the Haley House ecosystem, (program participants, Soup Kitchen guests, tenants, and Haley House staff members.)
This is a full-time position that offers a competitive salary based on the successful candidate’s background and experience and includes excellent benefits, including health, dental, PTO, and 403(b).
Key Responsibilities:
- Collaborate with program teams to recruit individuals from the reentry and unhoused populations to receive case management services and participate in Haley House programs and services.
- Enroll individuals in case management and LiFT programs, provide orientation, and complete the intake process.
- Complete Individual Development Plans (IDPs) with clients to identify needs, address barriers, support asset development, and set goals.
- Manage a client caseload to provide reentry and stabilization services and promote retention of employment and housing.
- Partner with external agencies that offer workforce development, employment opportunities, healthcare, mental health services, and access to healthy and nutritious foods.
- Maintain up‑to‑date community resource information and contacts with programs that provide services to clients.
- Meet regularly with clients to discuss progress toward IDP goals and ensure program compliance.
- Communicate with service providers as needed to support each client’s continued progress.
- Complete all required documentation for each client on the caseload.
- Attend meetings and trainings within Haley House and with external partners.
- Track client outcomes and assist with completing monthly and quarterly reports.
- Support housing placement efforts by helping clients obtain necessary documentation for BHA and other housing agencies.
- Perform additional tasks as assigned by the Operations Director.
- Build relationships with CORI‑friendly employers to expand employment opportunities for clients.
- Provide optional case management services to Haley House Bakery Café staff who identify as returning citizens and/or individuals recently exiting homelessness.
Qualifications:
- Bachelor’s degree in Social Work, Human Services, Psychology, Sociology, or a related field (or equivalent experience).
- Training in trauma‑informed care, motivational interviewing, or harm‑reduction approaches.
- Understanding of reentry challenges, including housing barriers, CORI issues, employment discrimination, and stabilization needs.
- Familiarity with homelessness services, shelter systems, and community resources.
- Knowledge of workforce development programs and employment pathways for individuals with CORIs.
- Experience with public benefits systems (SNAP, MassHealth, SSI/SSDI, RAFT, etc.).
- Understanding of mental health and substance use supports.
- Experience working with returning citizens, justice‑involved individuals, or people experiencing homelessness.
- Experience collaborating with service providers such as housing agencies, healthcare partners, and workforce organizations.
- Experience in maintaining documentation and tracking outcomes.
- Strong communication skills with the ability to build trust across diverse backgrounds.
- Ability to manage a caseload and prioritize tasks in a fast‑paced environment.
- Crisis intervention skills and the ability to de‑escalate challenging situations.
- Cultural humility and the ability to work respectfully with individuals from marginalized communities.
- Organizational and time‑management skills.
- Ability to maintain confidentiality and adhere to ethical standards.
- Empathy and patience when supporting individuals navigating trauma and instability.
- Non‑judgmental approach to client engagement.
- Ability to travel locally for outreach, appointments, and partner meetings.
- Basic computer proficiency (case notes, email, databases, Google Suites), and experience utilizing a Customer Relationship Management system (CRM.)
