Connections Community Support Programs, Inc.

Connections is a 501(c)(3) not-for-profit organization that has been providing community-based housing, health care and employment services in Delaware since 1985. The mission of Connections is to help vulnerable and disenfranchised Delaware residents to improve their lives by providing a comprehensive array of affordable and accessible primary medical, mental health and substance abuse treatment, employment, and housing opportunities that help the individuals and families receiving the services to become accepted and productive members of their communities. Through our housing, employment and treatment programs, we will serve almost 10,000 unduplicated individuals in 2010 in 55 separate grassroots locations throughout Delaware, including New Castle, Kent, and Sussex Counties.

We are the largest provider of housing for persons with incomes at or below 30% of area median in Delaware. With more than 500 units of permanent housing, we will provide 180,000 nights of safe, affordable housing this year. In addition to housing, our services include outpatient mental health and substance abuse treatment; homeless outreach and emergency shelter; primary medical care; community-based case management; home health care; food; and employment assistance for people who have barriers to employment because of disability, criminal justice involvement, or low rates of educational attainment. The services we provide are available to anyone in Delaware who needs them, regardless of where they live, the extent of their problems, or their ability to pay.

Connections’ services are unique in that they address every aspect of an individual’s life and needs. Connections helps persons served to obtain and maintain employment, receive benefits and income, learn how to better manage their illnesses, gain independent living skills, and acquire education. Our programs promote stability and self-sufficiency for extremely vulnerable adults, allowing them to increase their skills and income, get a job, find and keep housing and improve their health outcomes.
