CarePath Family Initiative empowers youth and families in Greater Boston through mentoring, case management, workforce development, and specialized support services — helping individuals ages 5–23 build stability, confidence, and a clear path forward.
CarePath Family Initiative is a Greater Boston nonprofit dedicated to meeting youth and families where they are. Through five core programs — CarePath Mentoring, Family Case Management, WorkReady Youth, Young Parent Support, and CarePath Autism Support Services — we provide wraparound support that addresses the whole person and the whole family.
Our team brings lived experience and professional expertise to every relationship we build. We believe that with the right guidance, resources, and community, every young person can thrive. CarePath serves individuals ages 5–23, with a commitment to equity, inclusion, and culturally responsive care across Greater Boston.
