Community Health Center

The mission of Community Health Center is to improve the health of our community through the provision of equitable, patient-centered, high quality, compassionate health care to all, regardless of ability to pay. Community Health Center provides care to more than 20,000 individuals annually within a "complete medical home" model, where multidisciplinary, integrated health services are provided under one roof to assist patients in overcoming affordability, financial, linguistic, cultural, transportation and other barriers to care.
