Our central hub empowers Oregon and Southwest Washington communities by helping people identify, navigate and connect with the local resources they need. We are invested in addressing the social determinants of health in 40 counties, using a contact center, mobile app, online database; data analytics; a resource database that's continually updated; and regionally based community engagement coordinators.
