Grace Hospice of South Bend

Grace Hospice is a Community Health Accreditation Partner (CHAP) accredited agency, providing patient and family centered care that optimizes quality of life. Our care team works together focusing on the patient's needs in their home, home of a loved one, or in a long term care or assisted living facility. This care team is made up of a team of doctors, nurses, social workers, counselors, hospice aids, chaplains, and trained volunteers. We take our mission statement to heart and implement it in everything we do.
