CoachMe is a startup nonprofit filling a critical gap in our healthcare system - helping turn a diagnosis of diabetes or high blood pressure into step-by-step action plans patients can follow. Our mission is to address the health disparities that face low-income Americans.

We work with patients on Medicaid and without insurance. Our health coaches remotely support patients over calls, text, and video to build healthy lifestyle habits that can improve their lives.
