Adventist Health Hospice consists of dedicated, hospice professionals who provide support and guidance to patients with life-limiting illness and their families in a variety of settings or wherever the patient calls home. Our holistic care model promotes healing through an Interdisciplinary Team while preserving dignity and advocating for patient choice at the end of life. We focus on offering patient centered care guided by principles of dignity, kindness, and respect. Our Interdisciplinary Team includes a medical director, nurses, social workers, home health aides, spiritual care services, bereavement counseling, volunteer services and adjunctive therapies. Together, we make it our mission to walk with a patient along this journey while focusing on their wishes and the things most important to them. Our goal is to support and tailor our care to the unique needs and circumstances of each patient
