The Boulevard of Chicago serves medically and socially vulnerable people experiencing homelessness by providing medical respite care, behavioral health services, and housing support. The Boulevard embraces a compassionate, client-centered, and holistic philosophy of care; and through strong partnerships, promotes healing and stabilityvto break the cycle of homelessness.
Medical Respite (Recuperative) Care
Provides short‑term residential care for homeless adults who are too ill or injured to recover on the streets or in shelters, but who do not need to stay in a hospital. This includes a safe place to stay, meals, nursing care, help with medications, and transportation to medical appointments.
Integrated Physical & Behavioral Health Services
Operates an on‑site clinic (in partnership with health providers) delivering primary care, mental health counseling, and substance‑use support to address both medical and behavioral health needs together.
Housing Navigation and Advocacy
Helps residents secure stable housing by coordinating assessments, benefits enrollment, and housing placement, and continues support after residents leave the program to reduce returns to homelessness.
