To improve access to high quality, patient-centered primary health care targeted to the needs of medically underserved communities.

Develops and operates community health care centers providing primary care, behavioral health, dental care and social services;

Trains health care professionals and engages in health services research and policy development;

Works to address barriers to health equity and eliminate racial and ethnic disparities in health outcomes;

Uses health information technology to enhance quality, efficiency, care coordination, and access to health records;

Promotes community outreach and health promotion programs;

Encourages workforce diversity and professional development.
