Volunteer Posting:
The Human Health Project is seeking a motivated, experienced leader with a passion for healthcare to volunteer as Health Navigation Lead for our Healthcare Access for All initiative.
In this volunteer role, you will serve as the senior accountable leader across multiple program workstreams, collaborating closely HHP team leads, community collaborators, and the HHP members who drive volunteer operations, expansion strategy, and mission outcomes.
The Health Navigation Lead acts as the advisor for all programs which interface with patient community referrals, community health work, and essential health service collaborations which includes participating as a key senior manager in planning, developing, organizing, and directing health programs.
As health navigation lead, you will:
- Monitor the specific needs and demographics of the patient referred to us for community health services and engagement
- Connect volunteers with the organizations necessary to provide individuals with appropriate services efficiently
- Analyze the cultural, social, and economic factors influencing the health of our communities served
- Promote health equity and improving overall community well-being through measurable KPIs
- Spearhead education dissemination on health topics such as disease prevention, wellness practices, and more to empower individuals to make informed health decisions
- Collaborate and lead a diverse team of patient advocates, patient care navigators, and health professionals to also identify and dismantle systemic barriers within the healthcare system
- Facilitate strong communication and relationship-building within teams as you interact with a diverse range of stakeholders, from patients and healthcare providers to community organizations and policymakers.
- Track and analyze the who, what, where, and how of improving community health
- Launch collaborative approaches to community health improvements
- Establish and maintain effective program protocols, patient support protocols, and referral pathways
- Investigate interventions that work for the greatest impact on health and well-being for all
- Assess the overall health care needs of our users and develop long-term partnerships to address finding
- Create links between HHP’s partners, the Healthcare Access for All program initiative, and pre-existing community resources and referral programs (such as 211) to maximize community impact and promote health and healing
- Develop and lead in health care events, such as health fairs and educational sessions
- Help community members access and navigate HHP resources and empower individuals in their health journey
- Lead, develop, direct, and implement clinical and non-clinical activities that impact health care quality cost and outcomes.
- Identify and develop opportunities for innovation to increase effectiveness and quality
- Oversee fiscal responsibilities including maintaining volunteer credentials for medical reimbursement and leading grant-writing efforts in community navigation areas
In order to ensure success, we ask that you have the following:
- Relevant education in healthcare and/or business leadership
- Direct managerial experience in managing teams, programs, and outcomes at a high level
- Enough free time to commit to our cause in a meaningful manner
