Welcome Calls to Hospice Patients and Families

Welcome Calls are a one-time phone call made to newly admitted hospice patients and their families. The purpose of the call is to provide an additional layer of support, ensure they have everything they need following admission, and answer any questions or concerns they may have. During the call, the volunteer will also reinforce important information, including how to contact the hospice office and the availability of support 24 hours a day, 7 days a week. These calls help promote comfort, communication, and confidence as patients and families begin their hospice journey.

Sample Call Introduction

"Hello, my name is [Name], and I'm calling on behalf of [Hospice Name]. I'm just checking in to see how things are going since your hospice admission. I wanted to make sure you have everything you need, answer any questions you may have, and remind you that our team is available 24 hours a day if you need assistance. Do you know how to reach the office if a need arises?"

This provides a warm, supportive touch while ensuring patients and families feel connected to the hospice team.
