    You will build genuine relationships with individuals and families experiencing homelessness, develop individualized service plans, and walk alongside each person as they navigate the path from emergency shelter to stable, permanent housing.

    Position Summary

    Case Managers at LASH Haven are the primary advocates for every client we serve. You will build genuine relationships with individuals and families experiencing homelessness, develop individualized service plans, and walk alongside each person as they navigate the path from emergency shelter to stable, permanent housing. If you believe in meeting people where they are and you are energized by seeing clients succeed, this role is for you.

    Key Responsibilities

    • Conduct individualized needs assessments and develop service plans within 72 hours of each client's intake
    • Provide weekly one-on-one case management sessions for an assigned caseload of up to 50 clients
    • Connect clients to Coordinated Entry and housing placement programs including HELP of Southern Nevada, SNAMHS, and Inclusion Advocates
    • Assist clients with vital document retrieval — state ID, birth certificate, Social Security card
    • Support clients in accessing employment training, benefits enrollment, and educational resources
    • Arrange transportation assistance including RTC bus pass procurement for housing and service appointments
    • Enter all client interactions, service deliveries, and housing outcomes into HMIS within 24 hours
    • Provide 30, 60, and 90-day post-exit aftercare contacts for all clients placed in housing
    • Collaborate with Shelter Manager and Intake Coordinator on client care coordination
    • Participate in monthly outcomes reporting and Clark County compliance documentation

    Qualifications

    • Bachelor's degree in Social Work, Psychology, Human Services, or related field — or equivalent experience
    • Minimum 1 year of direct case management experience with homeless, low-income, or vulnerable populations
    • Knowledge of Southern Nevada housing and social service resources strongly preferred
    • Experience with HMIS or similar client data management systems preferred
    • Bilingual English/Spanish a plus
    • Strong written and verbal communication skills
    • Ability to maintain professional boundaries while building authentic client relationships
    • Valid Nevada driver's license and reliable transportation required
