Circle of Care Volunteers call home care patients or their caregivers each week to check on their needs prior to the weekend. Volunteers follow a phone script, assisting them to identify patient needs such as medication refills, supplies and/ or equipment that need to be addressed prior to the weekend. Circle of Care Volunteers contribute to a proactive approach in caring for terminally ill patients and their families. After training and education, the calls may be done from home.
