Description

Reporting to the Director of Social Services and Resident Engagement, the Social Services Coordinator will work with a roster of tenants to address barriers related to housing stability. In coordination with each household, the Social Services Coordinator will develop goals, provide advocacy, and problem solve issues that impact their leaseholder status. In addition, the Social Services Coordinator will be responsible for administrative team support in various areas and needs.

Salary Range: $60,000 - $70,000 commensurate with experience.

Job Duties include but not limited to:

  • At minimum two to three days per week, meets with assigned households on a regularly scheduled basis in their home, in the community, and in the office to address challenges impacting their housing stability. Meetings could be more frequent dependent on the needs of the households. Ensure completion of ongoing timely case notes recording service delivery; including but not limited to assessments, intakes, goals, referrals, etc.
  • Identify service needs and appropriate intervention(s) to assist the households with desired outcomes, including entitlements, arrears recovery/support, and other provide direct supportive advocacy, and/or outreach services.
  • Serves as liaison with property management to address repairs, lease signing, etc.
  • Advocates for and assists households in negotiating various systems, to ensure receipt of entitlements and maintenance of permanent housing.
  • Attends appointments as needed with families to ensure all issues are addressed.
  • Properly track and respond to repairs, arears, and subsidy renewals for their team.
  • Identify information, referral sources, community partners, and other relevant parties for Resident Engagement program.
  • Maintain familiarity with entitlements, housing programs, and other benefits available in order to support the team with their ongoing work.
  • Serves as a liaison with property manager and various social service agencies.
  • Advocates for and assists families in the negotiation of the service delivery system, to ensure receipt of entitlements and maintenance of permanent housing.
  • Participates in weekly staff meetings and client conferences as needed.
  • Attend case meetings with supervisor to review case files as scheduled.
  • Attend staff meetings as scheduled.
  • Maintain complete, up to date, and accurate tenant records in company data management system at all times
  • Monitor data entry to ensure the teams’ timely submission of notes, goals, intakes, assessments, referrals, etc.
  • Oversee the tracking and reporting systems for assigned team, including but not limited to all trackers, ERAP applications, One Shot deals, and housing subsidy recertifications.
  • Provide coverage and support for other Social Services Coordinators as needed.
  • Ensure that department set deadlines, requirements, and expectations are met on a constant basis.
  • Conduct regular surveys on tenant needs and collaborate with the Director of Social Services on how to respond.
  • Perform other duties as reasonably requested.


QUALIFICATIONS & EXPERIENCE:

  • LMSW or MSW preferred, or master’s degree in related discipline.
  • At least 3 years of experience working with homeless/formerly homeless families.
  • Extensive knowledge of various city social service agencies and community-based organizations related to housing and homelessness.
  • Bilingual a plus but not a requirement
  • Ability to work Monday through Friday, consistent 8-hour shift with an unpaid hour for lunch.
  • Ability to travel to households within the NYC area using public transportation. Ability to work successfully in a hybrid environment.
  • Strong proficiency in Microsoft Office required.
  • Ability to relate to people of diverse backgrounds.
  • Commitment to providing affordable housing for low-to-moderate income population.
