Description

Will provide support to members by doing weekly check-in as needed via members' preferred method of communication. Your role involves tracking data and supporting the member with planning and problem-solving issues that arise on their education journey. This role is also responsible for connecting the member to resources within the community.  

Responsibilities: 

Discuss barriers to support them in completing their certificate program of choice 

Discuss solutions and provide linkages to community resources 

Provide person-centered care for members to motivate and support members in achieving their educational goals 

Communicate with the Field Coordinators to ensure all necessary providers and supports related to the support of the member 

Ability to build rapport and work with people of different backgrounds 

Perform all other duties requested  

Qualifications: 

Minimum education requirement is a High School diploma 

Knowledge within the field of case management 

Skilled in time management, organization, and ability to problem-solve

Ability to work flexible hours
