Description

ROLE AND RESPONSIBILITIES

The duties listed below are intended as examples of the types of work that may be performed by a Youth Case Manager but are not all necessarily performed and do not include all specific functions and responsibilities that the Youth Case Manager may be expected to perform.

  • Provide in-depth, wrap-around case management services to a caseload of youth in placement and the community, referred through Youth Rehabilitative Services.
  • Perform needs assessment with each youth, create individual reentry plans, and conduct individual case management sessions with youth, both at intake and at regular intervals while cases remain active.
  • Coordinate services and actively place youth at relevant job training programs, housing sites/programs, employment sites/programs, and social service, substance abuse, and mental health treatment programs.
  • Assist in youth’s resource utilization by accompanying or transporting them to off-site programs, job sites, and social service providers.
  • Provide frequent follow-up contact and support via phone, and office visits, as well as home, school, or community locations (Statewide, mostly Kent and Sussex County).
  • Conduct Cognitive Behavioral Therapy sessions in a group and 1-1 setting; Assist youth with building skills to manage daily stressors, improve impulse control, and reduce negative behaviors.
  • Collaborate with YRS treatment teams and families to support individual reentry plans; attend monthly treatment meetings, aftercare planning; and transition circle meetings.
  • Assess and report on client progress and participation at regular intervals; Track and update client information using the agency’s database program; complete case notes and maintain regular contact with YRS regarding youth progress.
  • Accept and perform other work as assigned.

QUALIFICATIONS AND EDUCATION REQUIREMENTS

  • Understands and supports the Delaware Center for Justice and Restorative Justice Philosophy and mission.
  • Significant knowledge and experience related to justice-involved people, the social forces and systems impacting them, and the resources available to them required.
  • A bachelor’s Degree in human services, social work, or a related field is also required. Master’s degree preferred.
  • Experience in motivational interviewing, strengths-based case management, and/or Cognitive Behavioral practices is also preferred.
  • Background checks and clearances are required as per program contract(s).
  • Basic computer skills, including but not limited to the use of Microsoft Office, as well as Zoom/Skype and other distance meeting resources.
  • Ability to multitask and work both independently and with supervision, on-site and remotely.
  • Embrace an understanding of justice involvement that acknowledges the impact of institutional racism and the systemic oppression of BIPOC, women, and LGBTQ+ people; must share the belief that people can change in positive ways.
  • Must embrace teamwork, have a strong work ethic, and be a self-starter with a positive attitude, with a good sense of humor.
