Community Health Worker

Cultiva La Salud is looking for a dedicated Community Health Worker who can demonstrate cultural and linguistic competency. The Community Health Worker is responsible for interfacing with community residents in-person, by phone and e-mail interactions.

The essential functions include but are not limited to:

• Portraying a positive company image and engaging in professional and friendly communications with community residents
• Conducting health education and outreach to residents in the City of Fresno and Fresno County.  Health education and outreach will include door to door activities, tabling, working at events and other activities within the community.  
• Conducting contact tracing as assigned and coordinating isolation supports to qualifying applicants.
• Staffing vaccine events in Fresno and Fresno County. 
• Collecting data and information from community residents served confidentially.   
• Use database to document and track participants cases.
• Provide detailed case management to individuals and families accurately assessing family’s needs. 
• Coordinate care and services (including identifying and accessing resources and overcoming barriers community members may experience)
• Complete appropriate referrals through the completion of the individual household- based risk assessment. 
• Facilitate development of an individual and/or group action plan and goal attainment 
• Coordinate activities with clinical and other community services
• Provide ongoing case management for families as a follow-up to ensure that the family has connected with all resources needed. 
• All other duties as assigned.

Skills/Qualifications

• High School diploma or General Education Degree (GED); or one to three months related experience and/or training; or equivalent.
• Experience in Social Work.
• Understands the needs and is part of the community being served.
• Being bilingual (Reads, writes, and speaks English and Spanish) is preferred but not required. 
• Information Technology skills such as use of Laptops, Smartphones, Internet, Web-based applications systems, Microsoft Office, Word and Excel, etc. 
• Reliable transportation, valid driver’s license and insurance required on vehicle used for work purposes (employee must be listed on insurance). 
• Weekend and evening work are required per program and community needs.
