We provide comprehensive, compassionate care for patients with limited life expectancy. This involves a team-oriented approach that addresses the medical, physical, social, emotional, and spiritual needs of the patient. Hospice also provides support to the patient’s family or caregiver.
Our hospice program is designed to help people of any age, from children to the elderly. As a active partner in the We Honor Veterans program, we are committed to ensuring veterans have access to quality end of life care as well. We work with patients in their homes, nursing homes, assisted living centers or hospitals. The hospice team - which includes physicians, nurses, social workers, spiritual care providers, aides and highly trained volunteers - works with patients, their medical provider and family to develop the best care plan. Hospice care neither hastens or postpones the end of life, but recognizes it as a normal process. We seek to help patients and their families emotionally and spiritually prepare for this time, while maintaining our goal of helping people stay in their homes whenever possible.
